社区获得性肺炎患者血清瘦素水平变化及其与C反应蛋白
和降钙素原的相关性研究

【摘要】Objective To explore the significance of leptin in severity prediction and treatment of CAP by observing the changes of serum LEP level and its correlation with PCT and CRP in CAP patients. Methods 85 CAP patients diagnosed in our hospitaliged patients were enrolled. Fasting venous blood samples of the in-patients were obtained and centrifuged on the dayl, day4, day7 and day of discharge. The sampling serums were stored at80℃ for further analysis. LEP and PCT were detected by using ELISA and CRP by chemiluminescence. Meanwhile , the patient's conditions were evaluated by PSI. According to PSI, the severity of patients were divided into minor group, moderate group and severe group , control group was set up by selecting 46 healthy volunteers. Results The LEP level of CAP patients is higher than the normal. The LEP levels reduced with the control of the inflammation and the relieve of the disease. There are some significant differences among groups (P < 0.01). As for the patients of the studying guoup, the LEP levels have positive correlation with PSI on admission.In different CAP group stratified by PSI, the LEP levels have positive correlation with CRP,
PCT and WBC (P< 0.05). Conclusion As a index,LEP can reflec the severity of the infection. The level of LEP can evaluate the severity of CAP. Combined with PCT and CRP, LEP play an important role in the seriousness prediction and treatment of CAP.%目的:观察社区获得性肺炎(CAP)患者病程中血清瘦素(LEP)水平的变化,及其与炎症标志物降钙素原(PCT)、C反应蛋白(CRP)的相关性,探讨LEP在CAP严重性预测和治疗中的意义.方法:CAP患者85例入选,在患者入院第1、4、7天及出院日采集清晨空腹静脉血检测血清LEP、PCT及CRP水平,并对患者情况进行肺炎严重指数(PSI)评分,根据评分将患者分为高危、申危和低危组,同时选取健康人46例作为健康对照组.结果:CAP患者血清LEP水平明显高于健康对照组(P＜0.01).CAP患者在住院的第1、4、7天和出院日的血清LEP水平随着炎症的控制和病情的缓解逐渐降低,各时段间比较差异有统计学意义(P＜0.01).CAF患者第1天LEP值与PSI呈正相关,有统计学意义(P＜0.05).在PSI评分的分组中,LEP与同组的CRP、PCT、WBC之间均呈正相关(均P＜0.05).结论:血清LEP可以作为一种反映感染严重程度的指标,对CAP病情的轻重程度有一定的评估作用;血清LZP与PCT、CRP的联合测定对预测CAP的严重性和指导治疗有一定的价值. 【总页数】4页(3863-3866)
